Service Coordinators are responsible for assisting individuals as prescribed within the Individualized Support Plans (ISP), providing quality care to individuals, and increasing quality of life in keeping with Gray Residential vision, mission, and core values. Service Coordinators oversee the daily operations of a specific caseload of individuals receiving services as part of the Gray Residential Family Supports Program and are responsible for implementing all aspects of service delivery. A Service Coordinator’s salary is based on a minimum of 160 billable hours per 4-week billing cycle. Consistent attendance and the ability to independently manage a work schedule are general qualifications for this job.
- Provide quality care of individuals via Direct Support Professional’s responsibilities.
- Ensure individual’s services are following all rules and regulations as indicated in 460 IAC 6, the individual’s ISP, and all Gray Residential policies and procedures.
- Prepare and submit all required paperwork: daily support records, staffing census, ISP data tracking, MAR, etc.
- Assist individuals with planning weekly and/or monthly activity calendars.
- Transportation of individuals to community activities.
- Demonstrate/maintain professional internal and external relationships, cooperation and partnership with all Gray Residential employees, supervisors, and individuals.
- Gathering and submitting monthly paperwork including service/program documentation and vehicle checks.
- Ensure incident reporting and follow-up is completed following Gray Residential incident reporting policy.
- Communicate issues promptly with the Program Director.
Minimum Qualifications:
Knowledge
- Current CPR Certification / First Aid Certification
- High School Diploma or equivalent G.E.D
- One-year direct experience working with individuals with developmental disabilities
- Some supervisory experience is preferred, not required
Skills & Abilities
- Ability to work independently to ensure effective implementation of the individual’s ISP
- Ability to follow detailed plans accurately, including both oral and written instructions
- Ability to apply sound, mature decisions
- Ability to communicate adequately to complete required forms and reports
- Ability to bend, stoop, sit, stand, reach and lift items weighing at least 75 pounds

- At least 18 years of age
- Flexible schedule
- Valid driver’s license, current liability insurance and dependable vehicle
- Current negative TB (Tuberculin test)
- Acceptable criminal (State and County) history background checks
Relationships
This position works under the supervision of the Program Director. The Service Coordinator is responsible for maintaining frequent communication with the Program Director as it relates to schedules and services provided. This position also has direct contact with individuals receiving services, their families, other service providers, and community partners.
