Position Overview
We're seeking an experienced Nurse Practitioner to provide comprehensive primary care services in a skilled nursing facility setting. This role combines clinical excellence with a patient-centered approach, offering both professional autonomy and collaborative team support.
Key Responsibilities
- Deliver primary, urgent, and preventative care to residents in assigned skilled nursing facilities
- Conduct comprehensive initial assessments, physical examinations, and medication reviews
- Manage complex medical conditions and coordinate care with interdisciplinary teams
- Provide clinical leadership and collaborate with facility medical directors and staff
- Communicate effectively with families regarding treatment plans and status changes
- Ensure compliance with regulatory requirements and quality standards
Required Qualifications
- Current Kentucky Nurse Practitioner License
- Active DEA License
- National Nurse Practitioner Certification
- Clinical background in adult, family, or geriatric care
- COVID-19 vaccination (exemptions available for qualifying circumstances)
Preferred Qualifications
- Experience in geriatrics, internal medicine, or long-term care
- Strong clinical assessment and diagnostic skills
- Excellent communication and organizational abilities
