Clinical Transplant Pharmacist - Specialized Patient Care & Research Opportunity
We are seeking an experienced Clinical Transplant Pharmacist to join our prestigious transplant center, where you'll play a crucial role in providing comprehensive pharmaceutical care for solid organ transplant patients. Key Responsibilities:
- Provide direct patient care across the entire transplant continuum, from pre-transplant evaluation to post-transplant follow-up
- Optimize immunosuppressive medication regimens and manage complex drug therapies
- Participate in multidisciplinary selection committee meetings and treatment planning
- Conduct patient and family medication education throughout care transitions
- Monitor and manage medication therapies for transplant-related complications and rejection
- Serve as a preceptor for pharmacy students and residents
- Lead research initiatives and quality improvement projects
- Develop and implement evidence-based clinical protocols
Required Qualifications:
- PharmD degree from an ACPE-accredited College of Pharmacy
- Current Virginia pharmacist license or eligible for licensure within 90 days
- Completion of ASHP-accredited PGY1 Pharmacy Residency
- Strong clinical decision-making and patient assessment skills
- Excellence in interprofessional communication and collaboration
- Demonstrated ability to work independently with minimal supervision
Preferred Qualifications:
- PGY2 Solid Organ Transplant Pharmacy Residency
- Experience in transplant pharmacy or related specialty area
- Research experience and publication record
- Teaching and precepting experience
