Physical Therapist - Home Health Specialist (Rochester Area)

Essential Responsibilities:
- Conduct comprehensive patient evaluations and develop personalized treatment plans
- Deliver expert physical therapy services in patients' homes
- Perform diagnostic assessments of muscle, nerve, joint, and functional abilities
- Create and maintain detailed patient documentation and progress reports
- Collaborate with healthcare teams to ensure optimal patient outcomes
- Educate patients and caregivers on treatment plans and proper use of medical equipment
- Participate in care plan development and case conferences
Required Qualifications:
- Graduate degree from an accredited Physical Therapy program
- Current New York State Physical Therapy license
- Minimum one year of clinical experience
- Valid driver's license and reliable transportation
- Strong interpersonal and documentation skills
