Position Overview
As a Home Health Physical Therapist, you'll provide comprehensive physical therapy services to adult patients in their homes, developing and implementing personalized treatment plans while working independently in the greater Rochester area.
Key Responsibilities
- Conduct thorough initial evaluations and ongoing assessments of patients' conditions
- Develop and implement evidence-based treatment plans aligned with physician orders
- Perform diagnostic tests and functional ability assessments
- Document patient progress and maintain detailed clinical records
- Educate patients and caregivers on treatment plans, exercises, and equipment use
- Collaborate with healthcare team members to ensure optimal patient outcomes
- Recommend and train patients in the use of adaptive equipment and mobility devices
Required Qualifications
- Graduate degree from an accredited Physical Therapy program
- Current New York State Physical Therapy license
- Minimum one year of clinical experience as a licensed PT
- Valid driver's license and reliable transportation
- Strong clinical documentation and time management skills
- Excellent interpersonal and communication abilities
