Key Responsibilities

As a Physical Therapist (PT) Case Manager, your primary role will be to deliver top-notch home health services and provide leadership within the care team. Your responsibilities will include:

• Conducting initial and periodic client evaluations to determine care needs and implementing effective treatment plans.
• Collaborating with physicians, caregivers, and other team members to develop and update individual care plans every 60 days or as needed.
• Serving as a client advocate by addressing safety needs, assessing home environments, and reporting concerns such as abuse or neglect.
• Performing direct care for clients of varying ages and conditions, adhering to Minnesota’s Physical Therapy Practice Act.
• Providing education to clients, families, caregivers, and team members to ensure effective care delivery.
• Supervising and offering clinical oversight for licensed and unlicensed staff in delivering client care.
• Keeping up-to-date and accurate documentation while meeting regulatory requirements and payer standards.
• Managing time effectively to adapt to evolving schedules and unexpected changes.
• Running financial management of home health services to ensure cost-effective, high-quality care.

Required Qualifications

We’re seeking a motivated professional with the following qualifications and skills:

• Current Physical Therapist (PT) license in Minnesota without any restrictions.
• A minimum of two years’ experience in a healthcare facility or home care setting.
• Familiarity with Electronic Medical Records (EMR) systems and proficiency in basic computer applications such as Microsoft Outlook.
• A valid driver’s license, reliable automobile, and current car insurance.
• Critical thinking skills to make sound clinical judgments and prioritize tasks effectively.
• Strong interpersonal, written, and oral communication skills.
• Proof of full COVID-19 vaccination and negative TB test results.
• Must pass a background check and disclose any criminal history related to health services.
• Current CPR certification is highly recommended.

Physical Demands and Work Environment

• Ability to lift and transfer patients weighing 50–100 lbs. occasionally, and 10–50 lbs. frequently.
• Work takes place in clients’ homes and community settings, requiring adaptability to various environments and conditions.
• Safe travel in diverse weather conditions using reliable transportation.
