We're seeking a dedicated Physical Therapist to join our team in a newly renovated Skilled Nursing Facility in Lake Placid, NY. Key Responsibilities
• Implement and manage skilled physical therapy treatments as per physician orders
• Maintain detailed patient records, including evaluations, progress notes, and discharge summaries
• Collaborate with interdisciplinary healthcare teams to address residents' special needs
• Participate in care planning meetings and provide input for resident care plans
• Conduct and assist with in-service training programs
• Oversee physical therapy aides and manage documentation in director's absence
Qualifications and Requirements
• Graduate of an accredited physical therapy program
• Valid New York State Physical Therapy license
• New PT graduates are welcome to apply
• Two years of experience in physical therapy preferred (not required)
• Passion for geriatric and subacute rehabilitation
• Strong leadership and communication skills
