Position Overview
As a Care Management Social Worker, you'll play a crucial role in our interdisciplinary healthcare team, providing evidence-based care while ensuring effective resource utilization and positive patient outcomes.
Essential Responsibilities
- Conduct comprehensive psychosocial and environmental needs assessments
- Develop and implement integrated discharge planning
- Provide crisis intervention and therapeutic support services
- Coordinate community service referrals and resources
- Deliver patient/family education and support
- Supervise graduate social work student interns
- Manage ED patient navigation and care coordination
- Facilitate access to primary and preventive services
Required Qualifications
- Master's degree in Social Work from an accredited program
- Current LMSW or LCSW licensure in New York State (or license-eligible)
- Minimum 2 years of healthcare social work experience
- Strong background in counseling and discharge planning
- Excellent interpersonal and communication skills
- Proficiency in electronic medical records and Microsoft Office
