Responsibilities :
- Clinical Care :
Provide skilled nursing care, including wound care, medication administration, injections, and monitoring of vital signs.
Complete thorough, accurate visit notes for each patient encounter, documenting care provided, observations, and any changes in the patient's condition.
Establish strong, professional relationships with patients and families to build trust and enhance their understanding of the care plan.
Maintain clear, effective communication with the agencys back office, updating the team on any changes in patient condition, care needs, or ongoing concerns.
