JOB SUMMARY
Under the direction of a Licensed Masters Social Worker, the School-Based Health Center (SBHC) Case Manager is responsible for providing mental health and prevention services to students and their families who are eligible to receive services. These services include providing guidance, support and / or referrals to assist students with increasing their overall level of functioning in the areas of emotional health, academic, family life and involvement in their community. Additional services include conducting educational trainings, community outreach, classroom observations, and providing consultative support to the school's faculty members. Participates in a team-based approach to care in the Patient Centered Medical Home (PCMH). Willing to be reassigned to other school-based behavioral health sites / locations during times when school is not in session.
ESSENTIAL JOB DUTIES

- Establish positive case manager relationships with patients, family members, community partners, school staff, and SBHC staff. Engage in building rapport and transparency within the professional relationship. (10%)
- Facilitate referrals to internal and external services, including but not limited to behavioral health treatment, substance abuse counseling / treatment, primary care, and social support programs. Complete new patient documentation and verify insurance eligibility. (10%)
- Collaborate with healthcare and care team providers, educational leaders, community agencies, and other stakeholders to ensure continuity of care. (10%)
- Conduct brief assessments of patients' behavioral health needs, including therapy, substance abuse treatment, psychiatry, and other social determinants of health. (10%)
- Conduct regular follow-up visits to monitor patients' progress and adjust care plans as needed. Provide ongoing support and advocacy. (10%)
- Review and maintain client records for all required documentation. (10%)
- Assist behavioral health staff with scheduling, coordination of care, outreach, and discharge. (10%)
- Participate in team meetings, case conferences, and training sessions to enhance knowledge and skills in behavioral health case management. (10%)
- Co-facilitate behavioral health treatment groups and provide community / school education about services that are provided. (10%)
- Stay informed about agency policy, best practices, and evidence-based interventions in behavioral health and school-climate initiatives. (10%)
