This position is a key member of our team; they are the liaisons between our participants and our health care team and a vital part of what makes PACE special.

Reports to:
- Transportation Coordinator and/or Center Manager
Basic Job Duties:
- Work with frail elderly participants in the PACE Program
- Responsible for the attending appointments with PACE participants;
- Must enjoy working in a fast paced environment, able to multi-task, be flexible, work well on a team, and love working with the elderly.
Requirements:
- Current CPR certification, if needed we will provide.
- Preferred caregiver experience, nursing assistants or CNA are encouraged to apply.
