Position Title: Community Health and Outreach Worker
FLSA: Non-Exempt
Reports To: Population Health Manager

Primary Function:
Under the direction of the Population Health Manager, the Community Health and Outreach Worker will assist in community outreach to ensure patients are scheduled for Covid vaccines, HEDIS measurement appointments, and appointments required by the MCOs.
Principal Duties and Responsibilities:
- Contacts and schedules patients for appointments related to Covid vaccines, HEDIS measures, and MCO- related appointments.
- Builds working relationships and collaborates with outside organizations to achieve common goals
- Assists with closing gaps in care.
- Assists with emergency room and hospital follow up appointments
- Provides program information and patient education resources as needed to patients
- Assists in management of referrals and closing referrals
- Follows up by phone calls, letters or visit on missed appointments or referrals.
- Links patients with case managers of MCOs to assist patients with any case management needs
- Performs related duties as assigned.
Nonessential Functions:
- Participates in team/staff meetings; attends in service meetings and applicable workshops.
- Performs other duties as assigned.
Minimum Knowledge, Skills, and Abilities:
- High school diploma required.
- Community Health Worker Certification required
- Experience in Community outreach preferred
- Flexibility required with onsite and remote work
- Must have strong oral and written communication ability necessary to communicate with agency staff and outside officials.
- Bi lingual a plus but not required
Working Conditions:
This job operates in an outpatient office environment.
Possible exposure to various environments such as: communicable diseases, toxic substances, medicinal preparations and other conditions common to a hospital and medical office environment.
Assist with outreach in the community.
Physical Requirements:
- Ability to cope with stress.
- The person in this position needs to occasionally move about inside the center
- Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine and computer printer.
- The person in this position frequently communicates with patients and/or employees. Must be able to summarize and exchange accurate information.
- Must be able to lift or catch patient weighing more than 50 pounds in the event of emergency.
- Performs with frequent interruption or distractions.
- Adjust priorities quickly as circumstances dictate.
- Ability to judge distances and spatial relationships to perceive objects where and as they actually are.
- Cognitive ability to analyze, count, summarize and synthesize information from multiple sources.
Compliance
This position requires compliance with Greater Baden Medical Services ( Health Center’s) compliance standards, including its Standards of Conduct, Compliance Program, and policies and procedures. Such compliance will be an element considered as part of the Director of Risk Management’s regular performance evaluation.
Language Skills
Ability to read and interpret written or verbal documents and instructions. Ability to speak effectively to patients, employees and/or stakeholders of the organization.
Travel
Must be able to drive between Greater Baden Medical Services, Inc. locations.
