Master's Degree in Social Work; Preferred Clinical track.
Experience
3 years of clinical social work experience, preferably in a medical setting

Principal Duties and Responsibilities
Consults and follows patients in a community based Palliative Medicine program as assigned.Meets with family members and significant others to obtain a relevant psycho-social assessment and to assess current and potential medical and social drivers of health needs.Develops and implements a plan of care appropriate to identified problems. Shares resultant findings with the appropriate health care team members. Care plans will meet the patients and family's care goals and minimize burdensome care transitions and uncomfortable symptoms of their disease. Suggests and arranges for hospice care when appropriate.Provides counseling services including individual, family and group modalities and other recognized psycho-social therapies in assisting patients and family members.Provides interpretation of the patient's, significant others, and family member's behavior as needed. Offers suggestions for modifying behavior to enhance a therapeutic approach.Provides and facilitates timely completion of appropriate documentation for patient records.Assists team to coordinate referrals, appropriate levels of care, and the use of therapeutic community resources to ensure a safe and appropriate care plan as well as discharge plans.Provides as well as attends educational in-services for community organizations and providers on palliative and hospice care.Participates actively in problem solving to enhance the most efficient and appropriate plan of care.Coordinates with other Gilchrist Programs to build / develop a complete Palliative Medicine service line.Participates in palliative medicine program development in collaboration with the palliative medicine interdisciplinary team and other service lines.Other duties as assigned.All roles must demonstrate GBMC Values
