Position Overview: The NRLP Long-Term Services and Supports (LTSS) Program Assistant Director will be responsible for assisting with managing the day to day program operations as well as providing supervision and guidance to LTSS Community Partner Coordinators and LTSS Clinical Care Managers. Will complete program intakes and care plans as needed, specifically focused on members received through the Lowell Embedded Project. Will also provide oversight to enrollees' discharge plans from the hospital or emergency department as well as support the NRLP Program Director in supervision of the overall program daily operations. Will provide back-up coverage to open caseloads as needed. Will serve as back-up for the NRLP Program Director and Lowell embedded CPCs as needed.
Position Responsibilities: Responsible for assisting Program Director in daily program operations and participate in all program evaluations and quality improvement activities related to EOHHS' LTSS Community Partnership program and other NRLP-related activities as requested. Will provide information about the LTSS options to enrollees, including a description of the role of the LTSS CPC and the enrollment process. Ensure care plan uniquely represents enrollee, including a cohesive overview of the enrollee's strengths and challenges, interests and goals, and the accommodations provided to support enrollee decision-making. Complete Program Intakes/HRSN assessments for Lowell project members as well as overflow from standard referral process as needed.
Qualifications:
- Current MA RN License required.
- Three (3) years of Clinical Experience required. Behavioral Health experience preferred.
- Three (3) years' experience in community nursing.
- A minimum of two (2) years supervisory experience in healthcare/human services required.
- Minimum of three (3) years of experience providing care management in a health and/or human services organization required.
- Previous work experience working with persons with disabilities preferred.
- Proficient in the use of computer systems, including but not limited to; Windows based software application such as Word, Excel, PPT and Outlook.
- Must have reliable transportation and valid driver's license for community outreach and home visits.
- Ability to work well both independently and as part of a team.
Hours 40 hours per week, Monday through Friday
