Title: Clinician (HOME) Housing Outreach Mobile Engagement
Reports To: VP of Care Coordination and Engagement Services FLSA: Exempt
Program: HOME (Housing Outreach Mobile Engagement) Location: Valley Cottage
Salary Range: $50,000-$60,000 annually
Summary Description: The Clinician (HOME) is responsible for engaging and supporting individuals identified by Rockland County Department of Social Services as living with behavioral health issues, exhibiting a history of disengagement with formal clinical supports and lacking stable housing. The Clinician (HOME) will engage clients around therapeutic support and case management style work to enhance overall wellness and help to achieve or maintain stable housing.
Key Responsibilities:
- Provide direct service, to include: engagement, assessment, therapeutic services, accompanying the client to appointments, removing transportation barriers, and others as needed
- Provide supportive collateral services as needed, such as: residential and housing, care coordination, family education and support, referral to and coordination with other service providers
- Identify and refer clients to appropriate community resources for additional support
- Maintain a caseload of clients as assigned to the HOME staff
- Assist in crisis intervention and response
- Maintain a case record for each client that is up-to-date and complies with all agency, DSS, and other regulatory agency policies and requirements
- Document all direct and collateral services and interventions in a timely manner
- Complete timely and accurate reports as needed
- Attend and complete all required and assigned trainings related to position, program, or agency goal
Key Competencies:
- Ability to engage clients through the development of a therapeutic relationship
- Ability to manage crisis situations and adapt to new or changing conditions
- Comfort and familiarity with field work – providing services in client homes and the community.
- Good verbal and written communication skills
- Knowledge of wellness, recovery, alcohol and substance abuse, coping, and relapse prevention strategies related to medical and behavioral health
- Proficiency in a fast-paced environment and multitasking
Key Performance Indicators (KPIs):
- Providing billable services to members served by the program on a monthly basis
- Improving health outcomes for clients enrolled in the program
- Moving clients along the stages of change for identified barriers to health, wellness, and maintaining stable housing
- Connecting clients to formal medical and/or behavioral health providers and improving independent living skills.
Requirements:
- Master’s Degree in mental health field
- Two years of experience working with individuals with mental health or substance use disorders
- Experience working with diverse populations
- Valid Driver’s License
- Basic computer knowledge: typing, communication tools, scheduling tools and web browsers.
- Proficiency in Microsoft Office suite and database management systems.
- Fluency in English (Reading, Writing, Speaking).
- Must be cleared by applicable regulatory agencies as required.
- Legally eligible to work in the United States without sponsorship.
Special Considerations:
- Employee may be sitting, performing computer work for extended periods of time.
- This position may require a flexible work schedule according to program needs.
- Hours Per Week: 35
- Hours Per Week, Reporting Structure, and Location are subject to change per program needs
