Summary Description : The Housing Case Manager plays a vital role in providing comprehensive support to individuals in securing and maintaining stable housing. This position involves collaborating with clients, community resources, and internal teams to address housing needs and promote overall well-being.
Key Responsibilities :
- Conduct housing assessments and develop individualized housing plans for clients.
- Assist clients in accessing affordable housing options and navigating the application process.
- Collaborate with property management, landlords, and housing agencies to secure housing opportunities.
- Provide ongoing support to clients in maintaining stable housing and addressing any housing-related challenges.
- Facilitate housing-related workshops and educational sessions for clients.
- Coordinate with social service agencies, healthcare providers, and community resources to ensure holistic support for clients.
- Advocate on behalf of clients to address housing-related barriers and challenges.
- Maintain accurate and up-to-date documentation of client interactions, housing plans, and progress.
- Participate in interdisciplinary meetings and case conferences to discuss client needs and progress.
- Stay informed about housing regulations, policies, and resources to provide up-to-date information to clients.
