Pay Range : $24.23 - $26.37 / hr
Summary Description :
The Outreach and Recovery Specialist plays a pivotal role in supporting individuals on their journey to recovery from substance use, mental health, and other adverse life challenges. This position involves providing personalized support, advocacy, and guidance to empower clients to achieve their goals to enhance their overall well-being.
Key Responsibilities :
- Engage with individuals with substance use, mental health, or other life challenges to build rapport and establish a trusting relationship needed to navigate those individuals to other needed services.
- To work with the aforesaid clients to develop SMART goals as a bridge to longer term treatment and services for improved life outcomes.
- Collaborate with clients to develop and implement personalized recovery plans.
- Provide emotional support, encouragement, and crisis intervention as needed.
- Advocate for clients' needs and rights within the organization and in the community.
- Collaborate with multidisciplinary teams to ensure coordinated and holistic care.
- Assist clients in accessing and navigating community resources and support services.
- Maintain accurate and confidential documentation of client interactions, progress, and setbacks.
- Monitor and assess client progress in achieving recovery goals and adjust plans accordingly.
- Participate in regular training and professional development opportunities.
