What are the responsibilities and job description for the Health Mentor position at Greater Nashua Mental Health?
Greater Nashua Mental Health is looking for a Health Mentor to join our team! The Health Mentor provides support and education related to physical fitness, nutrition, and wellness to adults and transition-aged youth seeking recovery services from a severe and persistent mental health condition under the direction of the InteGreat Program. Provides individual and group activity instruction to promote and support healthy lifestyle changes.
Duties & Responsibilities:
- Delivers community-integrated InSHAPE Health interventions that promote and support healthy lifestyle habits through individual and group-based interventions.
- Provide direct client care to individuals for a percentage of time per week as appropriate for position and as directed by supervisor.
- Conducts and documents InSHAPE physical assessments within the first 30 days of participants' entry into the program, and repeats physical assessment every 3-6 months through duration of the program.
- Conducts outreach for clients within assigned caseload when disengaged in treatment, per the missed client appointment policy.
- Completes daily progress notes within 3 business days of service (with first day being date of service/completion).
- Maintains daily schedules in EMR.
- Works collaboratively with team members by attending weekly treatment team meetings and InteGreat department meetings. Attends portion of client meeting with InteGreat Provider, as needed.
- Attends individual supervisions with the Director of InteGreat and Senior Health Mentor as scheduled.
- Promotes the InSHAPE program and supports health mentors by attending and participating in community events.
- Performs other duties or special projects as required or as assigned.
Qualifications:
- Bachelor’s Degree in Health Science, Social Work, Rehabilitation Psychology, Education or related field
- Personal trainer certification within first year of employment
- Experience and/or training in motivational interviewing and motivational strategy building.
- Commitment to reducing health disparities and social stigma towards mental illness.
- Commitment to being physically active and promoting the holistic benefits of good health.
- Fluency in Spanish strongly preferred.
- Organizational and time-management skills.
- Knowledge of Microsoft Office products (Email, Word, EXCEL).
- Strong collaborating skills, and an ability to work effectively both independently and as part of a team.
- Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
- Excellent communication and interpersonal skills.
- Ability to multitask, and be flexible with regard to workload and assignments.
- Ability to exercise judgement.
- Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy
