What are the responsibilities and job description for the Team Coordinator, ACT position at Greater Nashua Mental Health?
Greater Nashua Mental Health is looking for and Team Coordinator to join our Assertive Community Treatment (ACT) Team! The ACT Team Coordinator oversees a team of approximately 7-10 clinicians whose primary responsibilities are to provide intensive community based mental health services to individuals who are diagnosed with a serious and persistent mental illness and co-occurring disorders. Responsible for the provision of assertive treatment and outreach services to interrupt the cycle of relapsing mental illness in order to reduce incidences of frequent hospitalizations, episodic or continuous substances abuse, homelessness and difficulties within the legal system.
Duties & Responsibilities:
- Delivers face-to-face services to consumers for a percentage of time per week as appropriate for position and as directed by supervisor. Utilizes evidence-based practices including but not limited to, Assertive Community Treatment, Illness Management and Recovery, Motivational Interviewing, Supported Employment, Therapeutic Behavioral Services and Medication Supports to tailor treatment plans based on individual patient needs.
- Conducts assessments of all cases referred to ACT. Determines the appropriateness of admission, provides clinical assessments when indicated, assesses need for future evaluations and review of interim treatment plan and coordinates referral for and reviews results of additional assessments.
- Coordinates discharge planning and admission planning with NHH social workers. Provides on-site NHH with face-to-face client assessments and participates in client chart review.
- Reviews consumer history of illness, assessment of past and current function, results of evaluations, and multi-disciplinary review to provides clinical input and formulation of ACT Treatment Plan. Creates treatment plan that identifies the specific problems to be addressed, target goals, and clear and measurable objectives to be achieved by providing services within specific timeframes. Reviews and updates the plan to ensure compliance with agency guidelines.
- Coordinates the Consumer's Individual Service Plan within the Agency and with external agencies. Functions as consumers' advocate in obtaining needed services both within the clinic and in the community. Ensures that ISP services are carried out when consumer allows.
- Develops and maintains knowledge of evidence-based practices including Assertive Community Treatment, Intensive Dual Disorders Treatment, Peer Support Model and Individual placement and Support-Supported Employment model.
- Provides education, supervision and consultation to ACT team, agency staff, external agencies and community at large as required by the ACT team leader, ACS/ACT Director and Medical Director.
- Develops and maintains knowledge of evidence-based practices including Assertive Community Treatment, Intensive Dual Disorders Treatment, Peer Support Model and Individual placement and Support-Supported Employment model.
- Participates at least weekly in ACT on call and the emergency services supervisors on call rotation.
Qualifications:
- Master’s Degree in Social Work, Psychology, Human Services or related field
- NH Licensed or license eligible preferred LICSW, LCMHC, MLADC
- 1 years related work experience
- Experience working with people diagnosed with severe and persistent mental illness.
- Demonstrated skill in crisis intervention.
- Ability to lead and guide staff. Flexible, willing to learn, and anticipates the needs of self and staff.
- Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
- Ability to develop, execute, and monitor effective treatment planning and to adjust services and interventions as needed.
- Able to work flexible hour as needed, including occasional weekends and holidays.
- Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy
