Greater Seacoast Community Health (GSCH) is looking for a passionate and dedicated Community Health Worker (CHW) to join our team! In this role, you will work closely with the Strafford County Public Health Network (SCPHN) to provide vital outreach, education, and resource connection to individuals and communities, particularly around COVID-19 resources, healthcare access, and social services.

What You’ll Do
- Conduct outreach and education on COVID-19 resources, testing, and vaccinations.
- Assist individuals in navigating healthcare systems, including insurance enrollment.
- Support community vaccination efforts and participate in local outreach events.
- Identify and address social determinants of health (SDOH) by connecting individuals with necessary services.
- Advocate for culturally competent healthcare services through provider education and resource coordination.
- Participate in coalition meetings and professional development opportunities.
- Maintain detailed and accurate documentation in electronic medical records (EMR) and state reporting systems.
- Education & Experience: Bachelor’s degree in human services or related field preferred; equivalent experience considered.
- Certification: NH Community Health Worker Certification preferred or willingness to obtain within the first year.
- Skills: Strong communication, outreach, and problem-solving skills with the ability to work independently and collaboratively.
- Technology: Experience with electronic medical records (EMR) and reporting software preferred.
- Commitment: Passion for community health and wellness, with flexibility for occasional evening or weekend work.
- Knowledge: Familiarity with the Strafford County health and social services landscape is a plus.
