Responsibilities:
- Conduct patient assessments and develop individualized care plans
- Monitor and record vital signs, symptoms, and changes in patient condition
- Collaborate with healthcare team members to coordinate and provide comprehensive patient care
- Assist with discharge planning and ensure smooth transitions for patients
- Provide patient education and support to promote self-care and wellness
- Utilize case management skills to advocate for patients and ensure appropriate resources are available
- Maintain accurate and up-to-date medical records in compliance with HIPAA regulations
Qualifications:
- A bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area
- Proven experience in patient service and care management
- Knowledge of various medical conditions, treatments, and procedures
- Familiarity with ICU, pediatrics, behavioral health, or other specialized areas (preferred)
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to prioritize tasks, multitask, and adapt to changing situations
MINIMUM REQUIRED EXPERIENCE
- Two years of experience working directly with individuals with behavioral health conditions (if serving members with behavioral health needs) or with an I/DD or a TBI
- For care managers serving members with LTSS needs: two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience, in addition to the requirements cited above. (This experience may be concurrent with the two years of experience working directly with individuals with behavioral health conditions, and I/DD, or a TBI, above.)
Skills:
- Proficient in conducting patient assessments and developing care plans
- Knowledge of vital signs monitoring and interpretation
- Familiarity with discharge planning processes and resources
- Experience in case management and coordinating healthcare services
- Understanding of medical records management and HIPAA regulations
