- Provide care for client in clients' home, ensuring their comfort and well-being.
- Collaborate with interdisciplinary teams to deliver comprehensive patient care.
- Educate client and their families about health conditions, medication management, and post-discharge care plans.
- Ensure compliance with HIPAA regulations to protect patient privacy.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the state of practice.
- Experience in a healthcare setting, particularly in long-term care or medical-surgical nursing preferred.
