Key Responsibilities:

• Conduct health assessments, including vital signs and blood glucose monitoring, for patients with obesity and diabetes.
• Provide patient education on disease management, medication adherence, and lifestyle changes.
• Coordinate care across the interdisciplinary team, ensuring seamless integration of services.
• Administer medications, vaccinations, and other treatments as prescribed.
• Participate in community outreach efforts, including health fairs and mobile clinics.

Qualifications:

• Associate or bachelor’s degree in nursing from an accredited program.
• Current RN licensure in the state.
• Experience in chronic disease management and patient education preferred.
• Strong organizational and communication skills, with a focus on rural healthcare delivery.

Job Types: Part-time, Contract

Pay: $25.00 - $35.00 per hour

Expected hours: 15 per week
