Description / Summary
Under the general supervision of the Human Services Manager - Behavioral Health and agency prescribers, and in collaboration with the Comprehensive Community Services (CCS), Outpatient Behavioral Health (OBH), and Crisis Intervention programs, provide behavioral health assessment, case management, and treatment of Behavioral Health clients.
Essential Duties and Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following :

A. Psychiatric assessment, treatment, and clinical case management.
- Carry out clinical case treatment planning and management for an assigned number of clients.
- Perform mental status assessment and suicide assessment of clients, as well as provide crisis intervention services.
- Develop, implement, evaluate and revise overall treatment goals and plans for an assigned number of clients.
- Document client progress in the permanent clinical record of the client according to established methods and procedures.
- Provide community-based psychosocial rehabilitation services to persons with mental illness, including client specific ADL teaching / training, assuring basic needs of
client are met (medical, dental, and / or financial).
Attend and participate in recovery team and interdisciplinary team meetings.B. Psychiatric and substance use disorder medical assessment and treatment.
Transcribe, administer, evaluate and record psychotropic medicines prescribed by the agency prescribers, according to policy. This may include IM injections and / orimplantation of long acting medications.
Evaluate and chart psychotropic medication effectiveness, complications, and side effects, under the training and supervision of the agency prescribers.Administer drug screening through urine and / or saliva collection.C. Pharmacy management
Assist in maintaining pharmaceutical and other treatments and supplies as required for psychiatric medical and general medical assessment and treatment.Establish and maintain proper storage of medication and other pharmaceutical supplies.Coordinating pre-authorizations and medication refill requests.Coordinate labs and gathering results.Assist in maintaining medication records.Assist in packaging and adjusting medications as needed for clients, under the direction of the agency prescribers.D. Physical Health Monitoring
Perform and assist in physical assessment, both initial assessment and on-going, under the supervision of the agency prescribers.Participate in referral of clients to medical care providers, as well as assist in providing liaison between agency and these providers.E. Provide backup coverage for Behavioral Health Nurse (CSP) as necessary.
F. Other duties as assigned by the Human Services Supervisor - Mental Health, Human Services Manager - Behavioral Health and Human Services Director.

EDUCATION and / or EXPERIENCE
Bachelor's Degree in Nursing.Registered Nurse (RN) CredentialsTwo years of experience working with behavioral health and / or psychiatric medications.Experience with or willingness to obtain training in addiction medicine.Working knowledge of clinical case management and psychiatric medical assessments and treatment.CERTIFICATES, LICENSES, REGISTRATIONS
