What are the responsibilities and job description for the Marketing Liasion position at Green Hill Senior Living and Rehabilitation?
OVERVIEW:
The Liaison serves as the main point of contact for a given hospital, account or referral source.
The Liaison will facilitate and coordinate patient referrals and admissions to the organizations
various Centers through relationship development with hospital health care professionals- SWs, CMs, DC
planners, Physicians, and other medical professionals. The Liaison will clinically evaluate
referrals for potential admission to the Center and assess the patients’ medical record for the purpose of
identifying clinical acuity, special needs and projected discharge plan. The Hospital Liaison will provide
the appropriate clinical information and financial information to Center internal Admissions Teams who
will thoroughly assess clinical and financial criteria prior to determining the referrals acceptance status.
The Hospital Liaison will identify, build and develop synergies within accounts to develop partnerships
and collaborations with facilities that will enhance clinical capabilities and patient care and or improve
referral volume and build census.
RESPONSIBILITIES:
- Visit Hospitals daily
- Create and develop working relationships with SWs, CMs, DC planners at the hospital.
- Serve as the liaison between our Centers and hospital health care professionals (Physicians, DC Planners, SWs, Coordinators)
- Complete daily rounds and evaluate medical charts where requested/appropriate, evaluating medical needs, financial resources and DC plan
- Establish partnerships within the hospitals to be seen and provide value-based activities (i.e.) Attend patient rounds, provide educational presentations, CEU events, present patient care feedback and clinical outcomes, establish consistent rounding schedule)
- Follow up on all acute readmissions from all Centers and provide an update to internal Admission teams
- Generate new business to support Center census by creating new and maintaining existing relationships with hospitals, physicians, DC planners, CMs, SWs, NH professionals and other medical professionals within the hospital
- Plan and execute in-service presentations to existing and prospective referral sources
- Promote Center clinical outcomes and capabilities to develop partnerships with key leaders in the hospital
- Identify areas of opportunity within the market for growth and expansion
- Serve as a resource to both internal (admissions teams) and externally (hospital and professional sources) by supporting admission processing needs in a timely fashion
- Coordinate events- internally at the hospital and externally at the Centers for hospital medical professionals collaborating with Center Admission team and Administrator to plan and execute the event
- Work with Corp Business Development to establish metrics, goals, and quarterly marketing plans/objectives
- Assist with the development of team annual goals and objectives
- Sound knowledge and understanding of payment systems (Medicare, Medicaid, HMO, commercial insurance, workers compensation, PDPM, ACO models)
QUALIFICATIONS:
EDUCATION / EXPERIENCE:
- 2 years of experience in an Admissions/Marketing position in healthcare environment
- Long term care experience required
- Knowledge of Medicare and 3rd party billing
- Knowledge of medical terminology
- Ability to discuss clinical evaluations with physicians and other care givers within facility
SUPERVISORY RESPONSIBILITIES:
- This position has no supervisory responsibilities
WORK ENVIRONMENT:
- This job operates in a health care setting. This role requires regular walking to various locations around the care center.
- Incumbent may be exposed to virus, disease and infection from patients and specimens in working environment.
- Incumbent may be required to work extended periods of time at a video display terminal.
- Incumbent may experience traumatic situations, including psychiatric, and deceased patients.
- Works beyond normal working hours and on weekends and holidays when necessary
