Job Functions and Responsibilities: Provide authorized tasks to a client per the schedule assigned on the Individualized Service Plan (ISP).
Follow standard precautions in the delivery of services to the client.
Follow the Agency’s Policy on Client Rights and Responsibilities, including HIPAA’s rules on privacy.
Report changes in the client’s condition to the Supervisor or designee immediately.
Document activities and submit paperwork as instructed by the Supervisor or designee, if applicable.
May serve on the Interdisciplinary Team and/or the Quality Assessment Performance Improvement Committee.
Maintain positive communication and cooperation with the client, family, and coworkers.
Comply with all of the Agency’s applicable policies.
Work the assigned schedule and comply with the Agency’s timekeeping policy.
Other duties as assigned by the Administrator/Supervisor, or designee, when the needs of the Agency or its clients dictate.
Job Qualifications:
Age: Must be at least 18 years of age or, if under 18 years of age, be a high school graduate or be enrolled in a vocational educational program
Licensure/Certification: Not required
Education/Training: High school graduate/GED, preferred
Experience: Six months of working as a Personal Attendant preferred
Skills/Competency: Must be competent to provide personal assistance services as demonstrated by experience or testing; must be able to follow directions; may have to use and/or operate durable medical equipment (DME)
For Primary Home Care: not be the legal parent, foster parent, or spouse of a parent of a minor who receives the service; not be the spouse of the individual receiving the services, except for Family Care services; and not be designated by the HHS Case Manager as “Do not hire.”
Environmental Characteristics/Health Status Requirements: Works in a home environment under various conditions and temperatures; clients might have pets; must be able to lift, transfer, and support clients who may be overweight or disabled; must be able to move about in the home, stand for prolonged periods of time, perform fine motor tasks, bend, stoop, and twist for the provision of personal assistance services; must have reliable transportation, and, if driving, valid and current driver’s license and auto insurance for the ability to travel locally; TB screening and Hepatitis profile must be negative
