Requirements for Admissions Nurse (LPN):
- An unencumbered nursing license in the state of VT, LPN/LVN.
- Must have, as a minimum, one (1) year of experience with demonstrated success in a nursing position in the facility or in another related health care facility.
- Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to long-term care.
Major Duties and Responsibilities
- Collaborates with the Administrator, Director of Nursing and/or Assistant Director of Nursing to meet facility goals for occupancy and collections.
- Assists in converting referrals to admissions as directed by the Administrator, Director of Nursing and/or Assistant Director of Nursing.
- Supports discharge planning with Social Worker/Planner.
- Tracks referral status and communicates with external team members for necessary information.
- Reviews medical approval and medication availability for referrals, consulting with nursing leadership as needed.
- Transcribes physician orders, ensuring accuracy before admission.
- Ensures all admission paperwork is completed.
- Evaluates Medicare and Medicaid eligibility before admissions.
- Maintains knowledge of Medicare/Medicaid criteria and ICD-10 CM coding.
- Assists with timely referral processing and manages ACO audit logs.
- Identifies diagnosis codes for admissions and readmissions.
- Knows prior authorization requirements and secures approvals.
- Cross-trained in nursing department functions as needed.
- Communicates with the interdisciplinary team for clinical needs and resolves admission obstacles.
- Participates in performance improvement committees and follows up on regulatory inspections.
