JOB SUMMARY :
This full-time position leads and is responsible for the overall function of a collaborative, highly skilled, residential care team that provides support to a young man with multiple specialized (medical and behavioral) care needs. This position is ideal for an accomplished supervisor who believes that all people can contribute meaningfully to their community. Exceptional training, mentoring, and ongoing support is provided to all members of this team.
Schedule : The positions general schedule is weekday days, but the position will require some evening, overnight, and weekend work.
Location : Most work is performed onsite in Lamoille County but some limited work may be performed remotely.
ESSENTIAL FUNCTIONS :
- Staff development and supervision Trains DSP team in person-specific guidance and provides ongoing coaching and mentoring to ensure effective support of the individual. Provides supervision to all DSPs on the team. Coordinates and supports training provided by the Nurse and Communication Support Specialist.
- Staffing oversight Oversees the Direct Support Professional (DSP) staffing schedules; approves time off requests; arranges coverage when staff call out;and provides substitute coverage if no other staff are available.
- Leadership Ensures the team is functioning in an effective and collaborative manner. Sets a leadership example by demonstrating a continuous learning mindset, respect, and commitment to excellence.
- Medical Care - Ensures coordination of all medical care; attends all medical appointments; performs Special Care Procedures including insulin administration; prepares and updates Medication Administration Records; ensures availability and security of medications; and maintains strong communication with agency nurse and medical providers.
- Direct Support Serves as regularly scheduled second DSP 1-2 times per week. Provides substitute staff coverage for vacancies that cant otherwise be covered.
- Facility Ensures team members maintain the site properly; addresses or reports maintenance concerns; maintains the site with needed groceries and supplies.
- Service Coordination performs service coordination duties for the individual served, including plan writing, data collection and review, etc.
- Promotes the individuals autonomy, dignity, and choices across environments.
- Provides personal care including meal preparation and assistance with activities of daily living.
- Assists the person to participate in his community and make authentic connections with community members.
- Supports the individual to communicate effectively through careful observation, attentive listening, and by using his preferred communication tool and approaches.
- Monitors and supports the individuals health by preparing meals according to his meal plan, observing for signs of illness, administering medications, and providing diabetes care.
- Supports the individual to effectively manage emotions and cope with stress.
- Nurtures the person to develop and enjoy high-quality personal relationships.
- Completes work-related documentation in a timely manner, as instructed.
- Promptly communicates progress, concerns, and questions to the appropriate team members in support of excellent support provision and good team function.
- Attends team meetings and trainings as required.

Work is normally performed in a climate-controlled home environment and in indoor and outdoor community settings such as stores, pools, and walking trails. Work may involve exposure to aggressive behavior and bodily fluids / waste. There is a risk of needle stick injuries.
PHYSICAL DEMANDS : (The physical effort generally associated with this position.)
Work may include occasional pushing, pulling, or carrying objects weighing approximately 50 pounds. Work may include physical assistance to the individual to maintain balance or get out of the water. Work sometimes involves driving.
