Home Care Nurse Liaison - $30,000 Sign-on Bonus!
Salary Range : $42.64 - $65.01 Hourly
Work Type : Full-Time (40 hours / week, Day Shift)
Work Schedule : Monday-Friday, with some rotation

Responsibilities :
Record referral data for patient care from various sources into the appropriate log books and computerized systems.
Develop expertise in accepting or refusing cases, including understanding Medicare, private insurance, HMO, Title 19, and all contracted payors. Refused cases will be reviewed with a supervisor.
Notify the appropriate program supervisor of new referrals for Home Health Nursing, HHA, Therapy, Maternal Child Health, or Social Work services.
Review potential referrals for home care suitability by assessing facility patient records, consulting with facility staff, physicians, and patients or family caregivers to ensure smooth transitions from hospital to home.
Attend hospital rounds and initiate medication reconciliation, supply / equipment ordering, patient education, and other necessary components for a successful home care transition.
Required Qualifications :
RN License in Connecticut
Minimum of 1 year of experience in home health or hospital case management / discharge planning
Strong knowledge of managed care operations, including Medicare, Medicaid, and other payer sources
Ability to make independent nursing judgments and manage a high degree of public contact
Proficiency in computer data input / keyboard operations
