About the Role
We are seeking a compassionate and highly organized Case Manager to provide support to Greenbrook’s patient population. This role will manage Greenbrook’s transition of care program, working closely with our physicians to ensure our patients are supported through their hospitalizations and follow up care. The ideal candidate will also lead a program supporting our high need patients, and will serve as an important leader of Greenbrook’s quality program.
Location: This is a central operations position that is primarily work-from-home but the individual should be located in the Greater Tampa Bay / St. Petersburg area for occasional travel to our clinic locations.
Hours: Mon - Fri 8am-5pm

Key Responsibilities
As a key member of Greenbrook Medical, you will play a vital role in supporting our high-risk patient population through comprehensive case management and care coordination. Your responsibilities will include:
- Case Management: Assess, plan, and implement targeted interventions to address the unique needs of high-risk patients, ensuring they receive the necessary support for optimal health outcomes.
- Care Coordination: Collaborate closely with Greenbrook’s physicians and clinical staff to facilitate seamless transitions of care and provide holistic patient support.
- Hospital Discharge Oversight: Lead data management and oversight of Greenbrook’s hospital discharge program, ensuring smooth transitions and ongoing care for patients post-hospitalization.
- Patient Engagement: Proactively engage patients facing complex medical and social challenges, developing individualized care plans to address their needs and improve their well-being.
- Quality & HEDIS Program Management: Oversee Greenbrook’s HEDIS and quality initiatives, driving continuous improvement and ensuring the highest standards of patient care.
- Community Partnerships: Establish and maintain strong relationships with healthcare systems and key community organizations to enhance patient support and access to essential resources.
- Experience:
- 2 years of experience as a case manager
- Experience in managed primary care
- Certification / Education:
- LPN or background in nursing
- Skills:
- Ability to multitask, work independently, and influence others while working collaboratively with a diverse team
- Strong knowledge of HIPAA and other regulations related to healthcare administration
- Excellent communication, decision-making, and problem-solving skills
- Strong organizational skills and attention to detail
- Bilingual in Spanish is a plus
- Experience with EHRs, including ECW and Epic
- Familiarity with Google Suite and similar tools
