What are the responsibilities and job description for the Evening Nurse Supervisor position at Greencroft Communities?
The Evening Nurse Supervisor is responsible for leading, supervising, and resolving concerns that occur during the evening shift of skilled nursing services in the campus. To provide consistent high-quality care through leadership, supervision and support of the nursing team in identifying resident needs and providing quality clinical care.
Essential Functions:
- Oversee Greencroft Goshen team members giving direct care to residents and oversee the implementation of care plans.
- Supervise the admissions, discharges, and transfers of residents as assigned.
- Participate in developing a program of individualized care of residents. Initiate nursing care plan upon admission.
- Consult with ADON, DON and physician for special needs of residents.
- Communicate to other team members, family, and physician of any change in resident condition, and treatment of problem, as is deemed necessary and appropriate.
- Assist clinical nurses to problem solve, develop solutions, work efficiently and develop time management skills.
- Assure that incident investigations and reporting are completed according to ISDH and Greencroft Guidelines for Skilled Nursing.
- Train clinical team for adherence to state regulations and guidelines, Greencroft policies and procedures relating to clinical care of our residents.
- Will be responsible to provide coverage for ON CALL rotation that includes clinical and staffing.
- Acknowledge and resolve customer (residents, families, and team members) concerns.
- Complete routine rounds and address team member, safety, resident, and clinical concerns.
Requirements:
- Licensed as a Licensed Practical Nurse in the State of Indiana.
- Experience in Long Term Care and/or assisted living.
- Knowledge of MDS process and care planning.
- Experience with electronic medical records.
