Under the general supervision of the Program Manager or Supervisor, conducts Child and Adolescent Needs and Strengths (CANS) assessments; coordinates and facilities Family Team Meetings (FTM) or other family meetings; searches for and engages kin for children in agency custody, evaluates Kinship homes as a possible placement resource, and provides support, assistance, and referrals for services. Collects evaluation data, develops program components, and tracks outcomes for continuous quality improvement and program development.

EXAMPLE OF DUTIES
Performs related duties as required; any one position may not include all of the duties listed, nor do the essential functions cover all the duties which may be performed or required. Completes pre-conference activities associated with FTMs or other family meetings. Coordinates and facilitates FTMs and other family meetings in a manner that provides an overview of the purpose and process, an opportunity for participant involvement and questions, and a safe, supportive environment that encourages open communication. Assists with development of intervention strategies as a result to address identified barriers and gaps in services. Assures that FTMs are conducted according to agency/state guidelines, time frames, and expectations. Facilitates a quality decision surrounding child placement/custody which protects and provides safety in the least restrictive manner possible. Develops and maintains professional relationships with families while treating them with courtesy, respect, dignity, empathy and as individuals. Searches for relatives/kin for children in agency custody throughout the life of the case; attempts to engage relatives/kin in establishing relationships with the child (if appropriate) and assessing the relative/kin’s ability to become a placement resource for the child. Completes home studies for relatives/kin. Continually assesses children and families for abuse and neglect and safety in all settings. Identifies strengths and needs and engages family/kin in a strength-based, family-centered process. Advocates for kin; provides education on kinship rights, what actions they can take, how to work with the agency, and how the agency can better serve them. Conducts CANS assessments by coordinating the child’s team and connecting with the child, family members, substitute caregivers, and providers. Documents and tracks CANS assessment completion, due dates, review dates, court filing/action compliance, system documentation, and communication channels. Assesses the specific needs of children; delivers or coordinates the delivery of services. Refers children, parents, family members and/or substitute caregivers to supportive community resources for services (including but not limited to) childcare, mental health, substance use disorder treatment, homemakers, emergency, crisis intervention, etc. to help them address issues that may be placing children at risk. Coordinates and develops services with county partners to better support families and caregivers. Identifies additional resources, based on needs and gaps. Assures that documentation and case management records are complete, accurate, and properly distributed. Maintains accurate and comprehensive documentation and reports within required timeframes. Collects and generates reports to analyze and evaluate program effectiveness. Tracks outcomes for continuous quality improvement and program development. Recommends and implements program-improvement strategies. Prepares case testimony and documentation for court hearings. Collaborates with representatives of the judicial system and prosecuting attorney on case-related matters. Tracks relevant dates and actions to ensure compliance. Knows and adheres to applicable agency procedures, county policy, and state and federal rules and law*. Protects the client’s right to privacy and confidentiality. Presents to agency personnel, clients, and the public in a professional, competent, and culturally sensitive manner void of discrimination, sexual harassment, exploitation, coercion, manipulation, abuse, neglect, impaired practice, and conflicts of interest. Provides culturally competent services to clients, the public, and colleagues in a manner which is sensitive to the service population’s cultural and socio-economic characteristics. Establishes and maintains effective relationships with representatives of various social service agencies, county officials, court officials, law enforcement, and other parties. Develops collaborative relationships to provide team-oriented services and accomplish agency mission. Supports community activities that may have an impact on the agency. Encourages and promotes awareness within the community. Actively participates in agency meetings, including but not limited to case reviews, unit and division meetings, committee meetings, etc. Attends court hearings as applicable, preparing testimony and documentation. Participates in applicable policy and procedure revision. Participates as agency representative on external county and state teams and workgroups. Attends training based on identified training needs and utilizes concepts learned in training to improve practice. Provides training to agency staff and resource caregivers. May transport clients and/or children in personal vehicle. May function as ombudsman for the receipt of and response to complaints. *Understands and incorporates the agency’s Code of Ethics into practice. Performs other duties as assigned.

SUPPLEMENTAL INFORMATION
PHYSICAL REQUIREMENTS: Physical effort at the level normally found in office work or the equivalent. Although work is largely sedentary and involves sitting most of the time, it may involve occasional lifting and/or of small objects, picking up and carrying children, walking, standing, bending, stooping, crouching, kneeling, driving, reaching, fingering, keyboarding, grasping, talking, hearing, seeing, and repetitive motion. Exerting up to 50 lbs. of force occasionally and/or up to 20 pounds of force frequently to move objects, or constantly to lift, carry, push, pull or otherwise move objects, including the human body. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work, the job is rated for light work. Some outside work may involve going into homes where there may be a potential for exposure to hostile and dangerous persons or extremely poor home conditions. Incumbents may be subjected to verbally or physically aggressive clients and/or family members; threats, weapons, harassment, personal injury, physical violence, environmental factors such as fumes, noise, cold, excessive heat, dirt, contagious diseases, odors, bugs, rodents, aggressive animals, animal waste, human waste, trash, and body fluids while in client homes, meetings/appointments with clients, or transporting clients. Required to travel/drive in extreme weather (rain, snow, ice, etc.) which could cause hazardous road conditions. Will be exposed to outside atmosphere conditions as part of the job.PCN: 90330
