DESCRIPTION
To ensure that investigations are conducted in compliance with ODODD rule 5123-17-02 (Major Unusual Incidents) and 5123-17-03 (Abuse Registry), and appropriate sections of the Ohio Revised Code.
ESSENTIAL FUNCTIONS (ILLUSTRATIVE ONLY)
- Investigate (including face to face interviews) and provide appropriate reports on Major Unusual Incidents to include but not limited to, alleged abuse and/or neglect in accordance with department rules and guidelines. File MUI reports electronically with the State of Ohio Incident Tracking System (ITS).
- Establish and maintain an orderly process for the investigation and reporting of Major Unusual Incident and unusual incident reports.
- Ensure that all Unusual Incident Reports are investigated in accordance with the Ohio Revised Code and the Ohio Administrative Rules.
- Monitor patterns or trends that appear during the review of data collected during the review of Unusual Incident Reports and report those trends to the Superintendent. Follow up on unusual incidents with appropriate parties to ensure safety of the individual(s).
- Responsible for the Committee that reviews incident trends and patterns based on DODD rule, including assuring the required makeup of members, member training, scheduling of meetings and maintaining/distributing meeting records.
- Completes a quarterly review of provider incident logs per guidelines established in rule.
- Review provider semiannual MUI reports per guidelines established by rule.
- Provide technical support to crisis intervention plans and participate in follow up to review crisis situations.
- Provides technical training to staff of all appropriate agencies regarding their responsibilities to report unusual incidents.
- Attend appropriate training sessions to enhance investigator skills.
- Make presentations at GDBDD Board and other meetings as directed by the Superintendent.
- Arrange for involvement and work jointly with local law enforcement and County Departments when required by investigations.
MINIMUM QUALIFICATIONS
A minimum of an Associate’s Degree from a college or university in education, health services, human services, law enforcement, social or behavioral science, or a related field, and hold a valid Investigative Agent certification at the professional grade, issued by ODODD. Must successfully pass pre-employment requirements of: State and/or Federal Criminal Background Check; Nurse’s Registry check, ODODD Abuse Registry check, Office of the Inspector General check, Sex Offender Check, ODRC Offender Check, SAM Registry and employment physical including drug testing. Must maintain valid Ohio driver’s license in accordance with Board policy. Must successfully complete 300-day probation period.
REQUIRED KNOWLEDGE, SKILLS & ABILITIES (KSAS) (ANY COMBINATION OF EDUCATION AND EXPERIENCE)
Knowledge of: Methods of conducting investigations, ORC requirements for investigation and reporting unusual incidents. Ohio Department of DD rules on Major Unusual Incidents and Abuse Registry. Experience working with individuals with disabilities; Knowledge of disability rights laws and regulations; Knowledge of DD board policies.
Skills in: Conducting investigations; speaking to large groups; interpersonal relations organization; basic math; answering phones; typing; quality customer service; monitoring, collecting, and analyzing data on unusual incidents; preparing written reports on data collected.
Ability to: Analyze situations; identify facts and draw appropriate conclusions; handle multiple assignments concurrently; work under time constraints and meet deadlines; work with multidiscipline teams; recognize unusual or threatening conditions related to persons with disabilities; lift up to 25 pounds occassionally must have full range of motion in upper body; bending and lifting required. Stand, sit, or walk for lengthy periods of time.
Must be able to flex schedule when needed.
