What are the responsibilities and job description for the Care Manager position at Greene County Council on Aging?
The successful candidate will conduct phone and in-person assessments, set-up care plans, follow-up with provider agencies and provide ongoing care management to older adults and caregivers in Greene County. Other duties may include involvement with support groups, community events, and generating reports related to services, clients, etc. Candidates must be a licensed social worker and have a minimum of 3 years experience working with older adults and family caregivers. Candidates must also have a strong knowledge of in-home and community based services and be comfortable working in a team-centered environment. Excellent organizational, written and verbal skills are required.

Pay: $55,000.00 - $62,000.00 per year
