What are the responsibilities and job description for the Licensed Clinical Social Worker position at Greene County General Hospital?
The Licensed Clinical Social Worker position requires strong knowledge regarding Social Services and Care Management processes in preadmission, emergency department, outpatient/ambulatory services, and hospital acute short-term care, as well as post-acute care services. Also provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Other services include advising family care givers, providing patient education, counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
Essential Duties and Responsibilities
- Evaluates all Social Service referrals and identifies discharge planning needs, to include documentation of the patient discharge assessment in accordance with established guidelines and or hospital polices and processes; services to patients in the hospital acute care setting, outpatient services, and outpatient/emergency department.
- Collaborate with other professionals to evaluate patients' medical or physical condition and to assess patient needs.
- Refer patient and/or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
- Counsel patients in individual sessions to help them recover from illness, and adjust to life changing illness and events.
- Utilize consultation data and social work experience to plan and coordinate patient care and rehabilitation, following through to ensure service efficacy.
- Responsible for the facilitation and compliance in relations to CMS or state specific requirements or guidelines for PASSR forms for Skilled Nursing/nursing home placement.
- Coordinates end of life, palliative, and/or pastoral care services as appropriate for the terminal patient or end stage patient.
- Counsel family members to assist them in understanding, dealing with, and supporting the patient.
- Modify treatment plans to comply with changes in patients' status.
- Monitor, evaluate, and record patient progress according to measurable goals described in care plan.
- Identify social determinants of health through interviews and review of patient records.
- Develop or advise on social work policies.
- Assess and refer cases to Child Protection (CPS) as well as adults to Adult Protective Services (APS) as appropriate.
- Address Advance Directives – respond to inpatient as well as outpatient requests for implementation of advance directives – materials – completion of documents.
- Respond to crisis events within the hospital during social works routine workday.
- Provide Social Work coverage for Greene County Hospital Emergency Department as needed– assist with facilitation of behavioral health assessments and admissions.
- Complete inpatient behavioral health as well as chemical dependency assessments and suicide assessments as requested on inpatient admissions.
- Coordination of Swing Bed patients – admission, discharge, policy and procedures.
- Active participant in committees within Greene County General Hospital as well as in the community as requested – examples can include – Falls Committee, Opioid Committee, CPSI, PACE.
- Work collaboratively with case management nurse for facilitation of coverage for inpatients/discharge planning.
- Respond in a timely fashion to all referrals for inpatients, emergency room, and outpatient needs.
- Demonstrates clear ownership of workplace and patient safety.
- Reports mistakes, near misses, adverse events and quality and safety concerns.
- Participates in the development and implementation of safety and quality improvement activities.
- Protects patient privacy and confidentiality.
- Other duties as may be assigned.
Job Requirements
Education: Masters degree in Social Work required.
Certification/Licensure: Licensed in Clinical Social Work, required.
Experience: Experience preferred, but not required as a Licensed Clinical Social Worker. Knowledge of federal and state programs that provide medical care and financial support to patients and knowledge of community resources, preferred. Ability to work with diverse patient and staff populations.
Physical Requirements: Frequent sitting, standing and walking. Ability to reach, stoop, bend and use hands to finger and feel. The ability to occasionally lift up to 25 lbs. Good visual acuity.
