Wellness Program Director

Job Responsibilities:

• Timely and effective response to changes in health status
• Medication management and medical record maintenance
• Leadership in interdisciplinary-based care and training for team members and families
• Conduct pre-admission screening and ongoing assessment of resident's physical and psycho-social status
• Develop Individualized Service Plans and manage safety and infection control processes
• Communicate medical information to family, staff, and physicians
• Lead QA and compliance initiatives related to clinical matters
• Manage ongoing Wellness Program and deliver quality nursing care through daily rounds
• Act as Resident advocate in medical situations and collaborate with medical professionals
