- Start - ASAP
- Shift - 5 day workweek, 8:00 am-5:00 pm rotating on-call
- This position is for OUTPATIENT care.
- Must have passion for palliative care, hospice and the geriatric population
- Practice autonomously and provide symptom management of really sick patients
- Symptom management, chronic disease management, goals of care, advanced care planning
- 100% inpatient, round on inpatient hospice patients M-F
- Hospitalists admit, Open ICU
- Board Certified NP - REQUIRED
- Geriatric Certification (or willing to obtain within 1 year) - REQUIRED
- OH license (or willing to obtain) - REQUIRED
- Three years or more of work experience as NP in an acute care setting or with Palliative Care and Hospice (NO EXCEPTIONS) - REQUIRED
- Base salary plus w/RVU competitive salary range based on experience and training (to be discussed at interview).
- 200 hours of vacation/CME, 6 days paid holidays, birthday pay, sick leave accrual up to 600 hours
