Administrative and clinical activities necessary in the provision of screening, assessment and referrals, up to and including individual, group counseling and other family rehabilitative counseling services. Work is performed under the general supervision of the Community Based Services Coordinator. Provides a majority of services off-site at school and in-home locations. No supervisory responsibilities. May require extended after hours and weekend work.
Duties:
- Plans and conducts individual, family or group counseling sessions with clients exiting residential or juvenile justice facilities and/or clients at risk for incarceration.
- Conducts Behavioral Health and Substance Use Assessments and makes referrals as necessary.
- Comply with grant policy and procedures including: Provision of case management contact, urine drug screening and home/school visits at specified frequencies; Provision of alcohol and other drug treatment as appropriate to individual client need; and Arrangements for and monitoring provision of contractual wrap-around services.
- Provides individual therapy to adolescents with behavioral health and substance use issues.
- Provide a comprehensive array of specialized services, including family-based support, intensive case management and continuing care, as well as general attention to primary healthcare needs.
- Provides crisis intervention services as needed.
- Maintains up-to-date written documentation of client contact following documentation guidelines.
- Makes referrals to external agencies or internal programs as appropriate for the identified client.
- Participates in weekly treatment staffing to receive and provide feedback on client admissions.
- Maintains effective relationships with agencies via case management and community liaison activities/meetings.
- Maintain a caseload of no fewer than 18 clients or 80% of his/her capacity (minimum) and spend at least 60% of his/her time in direct client service activities.
- While position will be primarily responsible for Bridge Program clients, counselor may periodically be asked to assist with the provision of services at other locations as needed.
- Tracks measurable outcomes and meets program requirements as specified by The Bridge Program Grant.

Bachelors degree in human service area from accredited institution with successful completion of ADC process within 3 years of hire.
