POSITION SUMMARY: The Community Health Representative (CHR) is under the direct supervision of the CHR supervisor and the direction of the Medical Director. The CHR program is an outreach program and the CHR's act as a liaison, advocate for the Greenville Rancheria Tribal Health Program in our services areas. This is carried out through health promotion, screening, education, and activities.
General Duties and Responsibilities:
- Must be over the age of 21 years.
- High school diploma or General Education Development (GED).
- A minimum of one year knowledge of CHR duties (preferred).
- Be able to obtain a CHR certification to be a certified CHR through Indian Health Services (IHS) and able to pass re-certification through IHS every two years.
- To provide quality outreach health care services and health promotion/disease prevention services to "all" Greenville Rancheria patients within their communities using well-trained CHR's as mandated by Section 107 of P.L. 100-713, dated November 23, 1988, IHS.
- Must be over the age of 21 and possess a valid California driver's license and a DMV print out verifying a good driving record. Refer to the transportation/vehicle policy and procedure manual authorization of personnel.
- Occasional travel for meetings, trainings, and weekend outreach.
- This position requires an awareness and keen appreciation of all traditions, customs, and socioeconomic needs and the ability at all-times to meet and deal effectively with organizations, i.e., social services, etc. which requires tact, courtesy, discretions, resourcefulness, and good judgment in handling functions of a sensitive nature.
- Ability to pass a Pre-Employment drug screen and criminal background check/fingerprinting in accordance with all applicable laws regarding the nature of the employment.
- Requires current certification, or ability to obtain certification in Cardiopulmonary Resuscitation (CPR) and first aid.
- Must be a Certified Medical Assistant.

Direct Responsibilities
To be in the field every day to preform onsite home visits for "all" Greenville Rancheria * patients in our community/service areas.
Be able to drive a company vehicle from 9:00 am to 3:00 pm for home visits or projects. Refer to vehicle policy and procedure authorization of personnel.
Drive, assist "all" Greenville Rancheria patients to sign up for Medi-Cal for PRC. Report transport to the Medi-Cal Administrative Activities (MAA) program at CRIHB.
Act as a backup transporter.
CHR should be involved in several; preventative programs such as 1) car seat installation, 2) handing out smoke detectors and 3) handing out bike helmets.
CHR's are involved in the injury prevention program. CHR's perform home risk assessments on elderly Natives older than 65 or Natives that are less than 65 with a qualifying injury or disability.
CHR's are involved in coordinating the LIHEAP program for Natives of low income to aid with energy costs.
CHR's are required to take part in at least three community activities each for example, Gold Digger days in Greenville, The Red Bluff Rodeo Round-up parade and the Still Water Pow Wow in Redding or others as assigned.
CHR's should aid in the "Talking Circle", diabetes focus group for diabetes education, Native connections (suicide prevention), Teen Responsibility, etc. Other programs as assigned. For further details refer to the CHR manual.
Ensures compliance with legal issues including, but not limited to, patient confidentiality, risk management, ensures compliance with Health Insurance Portability and Accountability Act (HIPAA), Health Information Technology for Economic Clinical Health (HITECH), Federal, State, Local, Tribal laws and regulations.
