What are the responsibilities and job description for the RN Case Manager Hospice position at Greenwood Hospice?
Immediate Openings Available- RN CASE MANAGER FULL TIME in MERCER and MIDDLESEX Counties
Hours are 9am-5pm.
Greenwood Hospice is currently seeking qualified RN's to work as Hospice Case Managers to work in Skilled Nursing Facilities, Assisted Living Facilities and Patient's homes.
Must be able to perform all functions of an RN and be able to work with adult and geriatric patients and their families. The proper candidate must be a Registered Nurse licensed in the State of New Jersey with a driver's license and working car to provide case management of assigned patients under the supervision of the Director of Nursing.
Hospice and/or Homecare experience preferred. Position comes with a generous benefit package.

Qualifications:
- Active license to practice as an RN in New Jersey
- Minimum of 2 years of RN experience required with good references
- Reliable transportation
- Previous Hospice and or Homecare experience preferred
- CHPN a plus
