JOB SUMMARY
The MDS Nurse, Licensed Practical Nurse, LPN or Registered Nurse, RN is responsible for coordinating the MDS functions within the facility. Job responsibilities may include but are not limited to:
- Coordinate and implement the Resident Assessment Instrument (RAI) process in relation to all federal, state, and facility guidelines.
- Schedule and complete the Minimum Data Set (MDS) for residents.
- Be an active part of the interdisciplinary team.
- Develop, review, and revise the comprehensive person-centered plan of care.
- Be a resident advocate where the resident is the center of all we do.
- Ensure excellent communication among residents, families, and staff.
- Create positive resident and staff outcomes.
- Respect others by treating them the way we want to be treated.
- Demonstrate kindness, compassion, honesty, and integrity at all times.
- Help create a culture of superior customer service.
REQUIREMENTS
- Valid Licensed Practical Nurse (LPN) license or Registered Nurse (RN)
- Successful criminal background check
- Subject to pre-employment drug testing
- Long-term care facility experience preferred
- MDS experience required
