Home Health Registered Nurse – UAS Assessments
Job Overview :
We are hiring Registered Nurse Assessors for the New York Independent Assessor Program (NYIAP) . This is an excellent opportunity for nurses who want to learn how to conduct Uniform Assessment System (UAS) evaluations and transition into a role that combines clinical expertise with assessment-based care planning .
In this position, you’ll receive comprehensive training to conduct health assessments for Medicaid recipients (ages 19 ), helping determine their eligibility for long-term community-based services . Assessments are performed via telehealth and in-person home visits , with a flexible schedule based on consumer-driven appointments.
