Job Description:
We are seeking experienced and compassionate Licensed Clinical Social Workers (LCSWs) to join our team in both inpatient and outpatient settings within the VA Healthcare System. Candidates with VA experience are highly preferred. This is an excellent opportunity to provide critical support and care to our nation’s veterans while working alongside a dedicated interdisciplinary team.
Responsibilities:
- Conduct Biopsychosocial Assessments and create comprehensive care plans.
- Assess and address social determinants of health, mental health crises, and suicide risks.
- Facilitate safety plans, advanced directives, and discharge planning, including referrals to appropriate facilities or resources.
- Communicate patient data to providers and interdisciplinary teams.
- Provide crisis intervention and goals-of-care conversations, including family and interdisciplinary meetings.
- Assist veterans experiencing homelessness by providing resources and support.
- Identify and address cases of abuse or neglect in consultation with the appropriate authorities.
- Perform case management services post-discharge as needed.
- Maintain accurate and thorough documentation in patients' medical records.
Qualifications:
- Master’s degree in Social Work (MSW) from a nationally accredited school.
- Minimum of two years of current experience as an LCSW.
- Unrestricted LCSW license in any U.S. State, Territory, Commonwealth, or the District of Columbia.
- Proficiency in English (reading, writing, and speaking).
- Strong interpersonal, organizational, and time management skills.
- Physical ability to perform duties, including lifting up to 100 lbs, extensive standing/walking, and assisting with patient transfers.
Preferred Experience:
- Previous work experience in a VA healthcare environment.
Work Schedule:
- 40 hours per week with rotating shifts, including Federal holidays, based on staffing needs. Shifts are subject to adjustment as determined by the Department Director and Contracting Officer’s Representative (COR).
