Job Summary:
We are seeking a Certified Diabetes Educator to join our team. In this role, you will empower individuals with diabetes to manage their condition effectively through education and support. You will conduct individualized assessments, develop personalized plans, and provide comprehensive diabetes self-management education (DSME/T). This includes teaching healthy eating, active living, blood sugar monitoring, medication management, insulin administration, and the use of diabetes technology.
Job Duties and Responsibilities:
- Conduct thorough, individualized assessments for people with or at risk for diabetes.
- Collaborate with patients to set specific, measurable, and achievable goals.
- Develop and implement DSME/T plans aligned with patient goals.
- Evaluate outcomes using established standards, focusing on behavioral and clinical changes.
- Maintain accurate records of patient assessments, plans, and follow-ups.
- Integrate current technology into practice, including glucose monitors, insulin pumps, data-sharing platforms, and telehealth.
- Foster a positive, patient-centered environment and work collaboratively with healthcare team members and families.
Job Requirements:
Option 1: Registered Nurse background
Education:
- Associate degree or higher in Nursing
Licenses and Certifications:
- Active [Idaho] Registered Nurse license.
- Certified Diabetes Care and Education Specialist (CDCES) certification.
- Basic Life Support (BLS) certification.
Work Experience:
- Preferably CDCES-certified or with 2 years of experience working with people with diabetes.
Option 2: Registered Dietitian background
Education:
- Bachelor degree with specialized training in Dietetics
Licenses and Certifications:
- Active [Idaho] Dietitian and Nutritionist
- Registered with the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics
Work Experience:
- Previous experience in clinical dietetics
Knowledge, Skills, and Abilities:
- In-depth understanding of diabetes treatment and management.
- Strong leadership and group management skills.
- Proficiency in basic computer applications.
Functional Demands:
- Must be able to lift up to 40 lbs occasionally, 20 lbs frequently.
- Standing: Up to 4 hours continuously, 8 hours per day.
- Sitting: Up to 2 hours continuously, 4 hours per day.
- Fine motor skills for handling medical equipment and computer entry.
- Clear vision and communication skills required.
Environmental Conditions:
- Occasional exposure to biohazards (e.g., glucose monitoring, pump insertion).
