Primary Care Triage Nurse

Job Summary:
We are seeking a dedicated and compassionate Triage Nurse to join our Downtown Clinic team. This role is integral to assessing patient needs, planning care, and ensuring the best possible outcomes. The Triage Nurse collaborates with clinic representatives, coordinates transfers to the Emergency Department, and provides leadership in task assignment while mentoring peers and students. This position reflects leadership and professionalism without being a supervisory role, aligning with our mission and core values.

Key Responsibilities:

Patient Care and Triage:

• Assess walk-in patients to determine acuity and care needs.
• Collaborate with providers to ensure quality care and appropriate resource utilization.
• Coordinate with the Emergency Department for seamless care transitions.

Leadership and Coordination:

• Delegate responsibilities to clinical staff based on training, education, and availability.
• Utilize nursing knowledge to manage staff assignments and patient care priorities.
• Serve as a resource for communication and negotiation with patients, families, and staff.

Care Planning and Quality Assurance:

• Develop and routinely update patient care plans, adjusting to changing conditions.
• Conduct audits of patient records to ensure care plans and orders are accurate and complete.
• Identify and implement measures to improve patient care quality and safety.

Safety and Compliance:

• Ensure the clinical environment meets safety and accreditation standards.
• Check the functionality of equipment and address any deficiencies.
• Maintain strict compliance with confidentiality and organizational policies.

Professional Development:

• Mentor peers and nursing students to foster growth and collaboration.
• Stay informed about trends, developments, and best practices in nursing care.

Requirements:

Education:

• Graduate of an accredited school of nursing.

Licenses and Certifications:

• Current [Idaho] Registered Nurse license or compact state license.
• Basic Life Support (BLS) certification required.

• May work irregular hours or shifts in unpredictable situations
