Ground Floor Collective is seeking a dedicated and solution-focused Family Advocate to join our 100 Families team. In this role, you will recognize client needs, connect them with appropriate resources, and assist them in their journey from crisis to stability and onto a career. You will be an expert in community resources and 100 Families partnerships, maintain positive relationships with partner organizations, and advocate for the well-being of our clients.

Responsibilities:
- Conduct assessments to assist in determining relevant resources and guide individuals through the resource connection process.
- Develop strong, trusting relationships with clients served and act as an advocate for vulnerable individuals.
- Serve as a bridge between individuals and providers to increase client involvement in their care plans.
- Coordinate measures to ensure individuals have proper access to resource and treatment options.
- Maintain detailed documentation of work with each client, utilizing designated case management system as trained in order to comply with federal grant regulations.
- Update case management system on a regular basis to reflect client’s progress, in compliance with monthly reporting requirements based on performance indicators.
- Refer individuals to appropriate outside organizations; provide follow-up to ensure the individual’s needs are met and outcomes of these services provide support for the individual to achieve their goals.
- Empower clients to lead themselves and hold clients accountable to follow through on the plans they have developed.
- Advocate for clients in all areas of their life – provide support to clients at court sessions, DCFS hearings, and the like.
- Report back regularly to designated individuals regarding the client’s progression and participation.
- Must be comfortable working with clients in any of the following environments: jails, parole/probation offices, child welfare offices, courts, Family Shelters.
- Develop and maintain positive relationships with 100 Families partners and stakeholders.
- Develop a working relationship with other Family Advocates and community service providers.
- Participate in 100 Families meetings and trainings, including case management training provided by Restore Hope and weekly/monthly meetings requested by RH and/or the area coordinator.
- Promote organization and community collaboration.
- Respond to inquiries from a plethora of sources (e.g. individuals, organizations, providers) through offering information and/or direction.
- Conduct oneself in a manner consistent with the professional standards and values of 100 Families and Ground Floor Collective.
Preferred Qualifications:
- 3 or more years of work or personal experience working with vulnerable individuals, OR
- Bachelor’s Degree in social work, criminal justice, or a related human services field, and one year of work or personal experience working with vulnerable individuals.
- Bonus: work experience with the criminal justice population and/or knowledge of the criminal justice system.
