Position Purpose
Work with members that are enrolled in health promotion and wellness programs by providing health and wellness education, resources, and support through health coaching sessions.

Essential Position Functions
- Meet one-on-one with members enrolled in the wellness program and commercial health promotion programs through in-person, virtual, or telephonic appointments.
- Conduct health assessments by administering online Health Risk Assessments and completing biometric assessments that can include body measurements (height, weight, body circumference, blood pressure, body fat percentage). Lipid panel/glucose testing (via fingerstick test) and other relevant tests.
- Review and provide education on the results of health assessments.
- Develop wellness plans and care plans that addresses goals, barriers, action steps and interventions based on assessment results and members personal needs and interests.
- Complete required assessments in a timely manner.
- Ensure members understand the components of the health and wellness programs and how to earn any incentives that may be included as applicable.
- Use motivational interviewing to create behavior change. Identify barriers and provide the best possible solution to reach wellness goals.
- Educate members on health and wellness topics and provide resources.
- Provide support and tools to encourage self-management.
- Ensure member is receiving appropriate follow-up treatment, making lifestyle changes, and using self-management resources.
- Collaborate with co-workers and outside resources regarding continuity of care options.
- Maintains a professional approach with the highest standard of confidentiality.
- Maintain the standards of accurate and complete documentation and reporting.
- Work collaboratively with Quality Improvement, Case Management, and other departments to coordinate services for members.
- Maintain high employee morale, promote staff enthusiasm and teamwork, and assist co-workers in their job functions.
- Ability to work occasionally outside of normal business hours and travel to on-site locations based on department/company needs.
Minimum Requirements of the Position
- Bachelor’s degree in health education, exercise science, or a related field with major course work in health education, worksite wellness or a closely related field.
- Ability to conduct health risk assessments, plan and implement effective health education programs and evaluate the effectiveness of the comprehensive program provided.
- Excellent individual/group verbal and written communication skills along with strong organizational, planning and coaching skills a must.
- Demonstrated ability to be resourceful, creative, and innovative.
- Ability to effectively interact with all levels throughout the organization to be served.
- Demonstrated strong customer orientation.
- Shall abide by company policy and procedures to maintain compliance with Federal and State laws related to privacy, security, confidentiality, and protection of information resources and health care information.
- Ability to maintain the integrity of confidential employment and business information.
- Valid Driver’s License and use of own transportation.
- Proficiency in Microsoft Word, PowerPoint, Excel and Internet navigation tools.
