Job Overview: To provide supports necessary to enable persons with developmental disabilities to develop and maintain skills that will foster personal growth and independence. Act as liaison between community members and individuals. Act as an advocate to protect the rights and interests of the individuals served. Ensure fair and equitable treatment and provide quality opportunities for personal growth of the individuals served. Keep accurate program records on individuals served, track data as required and complete progress notes. Participate as a member of the ISP team as directed. Be an integral part of providing support and program based skills development. Work as team member to improve the effectiveness and efficiency of services provided.

Essential Functions of Position:
- Assist people served explore and identify meaningful recreational and social activities in the community.
- Facilitate with community integration.
- Provide training and support for people served in all activities of daily living, ensuring integration through facilitating interaction with community members.
- Participate alongside the people served in various community activities (recreational activities, volunteering, educational classes)
- Organize, develop and implement daily group or individual activities that help with community integration.
- Teach work related skills and behaviors such as appropriate communication, on-task behavior and appropriate social interactions
- Complete and maintain programmatic documentation as required.
- Assist and attend to personal care needs of the individuals, including hygiene, bathroom assistance, medical and clothing.
- Develop and maintain positive relationships with guardians, family members and other providers.
- Act as an advocate to protect the rights and interests of the people served.
- Attend to the daily needs of people served as well as fostering independence and self-determination.
- Assist with transportation to and from the program using agency vehicles.
- Ensure and promote ongoing health and safety of people served.
- Participate in carrying out support plans based on each persons needs and goals.
- Assist with behavioral interventions and follow through with behavior support plans for individuals assigned or as needed.
- Provide opportunities for learning and development of new skills.
- Be an integral part of providing support and service based skills development.
- Attend all required trainings and meetings.
- Maintain knowledge of ongoing program issues and developments as well as updating supervisor in a timely manner.
- Perform other related work duties as assigned.

EDUCATION and/or EXPERIENCE
Direct care experience working with individuals with disabilities. Associate's or Bachelor's Degree in human services or related field preferred. Demonstrated ability to work both independently and collaboratively with a cultural diverse population of individuals, managers, customers and general public. Ability to read and write clear, concise and accurate daily reports and documentation. Ability to record data using a computer
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Effectively present information and respond to questions from groups of managers, clients, customers, and the general public
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentages. Apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY
Ability to define problems: collect data, establish facts, and draw valid conclusions.
CERTIFICATES, LICENSES, REGISTRATIONS
Require valid drivers license and acceptable driving record. Pass Criminal Background Check (CORI). Current First Aid, CPR and Crisis Intervention certificates or be able to obtain these within thirty (30) days of hire. MAP certification preferred.
PHYSICAL DEMANDS
While performing the duties of this job, the employee is frequently required to stand; walk; use hands to finger, handle, or feel; and reach with hands and arms. The employee is occasionally required to sit; climb or balance; stoop, kneel, crouch, or crawl; and talk or hear. Ability to lift 30 pounds as needed.
WORK ENVIRONMENT
While performing the duties of this job, the employee is occasionally exposed to wet and/or humid conditions, moving mechanical parts, outside weather conditions, and vibration. The noise level in the work environment is usually moderate. The work environment characteristic will change depending on the location of services and supports being offered to the individuals.
