Description : PURPOSE OF CIE
Community Information Exchange (CIE) is a person-centered service that unites a network of Service Providers to address social and system barriers to services. The system was created as an interoperable data system to assess needs, facilitate referrals and care coordination, and document interventions and assistance for the health and social services sector to harness real-time data for proactive community care planning.
STATEMENT OF THE JOB
CIE Navigators will facilitate communication and resource-sharing among various community organizations, agencies, and stakeholders. This role also includes facilitating restorative justice and conflict transformation processes to resolve disputes and promote healing within the community. The position requires a compassionate, proactive, and strategic approach to problem-solving and community engagement.
ESSENTIAL FUNCTIONS
- Complete client intakes to identify the individual’s needs, identify resources to meet those needs, make close loop referrals to identified resources through the CIE database
- Utilize a comprehensive database of community resources and services to connect clients with appropriate support systems tailored to their unique needs and circumstances.
- Advocates for the service recipient, when necessary, to assist in contacting and utilizing resources.
- Conducts follow-up calls to ensure that the service recipients needs have been addressed or for additional resource connection support
- Uses crisis management techniques in accordance with agency procedures
- Maintain accurate and confidential client records and documentation in compliance with relevant regulations and policies
- Gain an understanding of how to navigate the full extent of services and processes that the CIE system can provide, which will be learned through in-house and hands-on training
- Manage relationships with partner organizations and develop systems to ensure coordination and communication between CIE and community partners’ staff
- Facilitate Gryphon Place restorative justice practices to address conflicts and harm within the K-12 school building and in community settings.
- Conduct and document restorative practices and facilitate conference and circle processes.
- Attends staff, team and department meetings and training as assigned
- Attends on and off-site education training relative to job duties as assigned
- Provides outreach services in community settings in the development and distribution of informational materials to increase youth, family, and community awareness of Gryphon Place services
- Other duties as assigned
