STATEMENT OF THE JOB
The Community Intervention Specialist provides culturally aware support and guidance to individuals involved in the Justice Lifted Diversion program. They are responsible for conducting assessments, developing intervention plans, and facilitating access to resources and services aimed at preventing recidivism and promoting positive behavioral change. The specialist works closely with youth and their family, community organizations, and other stakeholders to ensure effective implementation of diversion strategies and support systems. This role also includes facilitating restorative justice and conflict transformation processes to resolve disputes and promote healing within the community. The position requires a compassionate, proactive, and strategic approach to problem-solving and community engagement.
ESSENTIAL FUNCTIONS
- Develops a 90-day intervention / prevention plan for youth and family referred to the program; provide linkage with appropriate community programs, external agencies, re-engage youth in education, and / or prevent continued unsafe activity; respond to questions / inquiries related to program components.
- Facilitate Gryphon Place restorative justice practices to address conflicts and harm within the K-12 school building and in community settings.
- Conduct and document restorative practices and facilitate conference and circle processes.
- Create a supportive environment that encourages youth to express themselves confidently and contribute actively to the planning and decision-making process.
- In collaboration with the youth, work with other internal and external partners (i.e., schools, faith community, other human services organizations) to develop resource supports to eliminate barriers to services, enhance existing community services and / or develop new services based on the needs of the youth.
- Engage in street outreach to build relationships, provide resources, and connect individuals with community support services, as well as other street outreach professionals.
- Maintain accurate and up-to-date case records, documenting client progress, intervention strategies, and service referrals.
- Attends staff, team and department meetings and training as assigned.
- Conducts outreach in community settings in the distribution of informational materials to increase youth, family, and community awareness of Gryphon Place services.
- Performs other duties as assigned.
